  • Immunizations: The most recent copy of the student's immunizations and/or waivers.

  • Academic Records: The most recent copy of the student's report card, unofficial. 

  • Student Application: Each student applying for 6th-12th grades must complete an individual application as well.

  • References: Please provide 3 references per student applying for 6th-12th grade.
